- Feb 24 & 25 5 – 7pm

- Sports Physical MUST be completed prior to participation – See the Documents link in the menu for the form to take to your doctor or urgent care facility.

- Baseline Concussion test MUST be completed by the following students:

ALL 7th graders that have not already had it done this year for another sport

8th graders that did not have it done in 7th grade or are new to the school, and have not had it done earlier this year for another sport

How to complete the Baseline Concussion Test
1. Permission Slip: See Coach Daniels or Mrs. Whorley IMMEDIATELY
